Missed Appointment, Late Cancellation (less than 48 hours) or No Show Fee

I hope all is well with you.  We had an appointment scheduled.  I know that life can get really busy and I’m sorry that I didn’t get to check in with you.  I appreciate you taking care of this charge. You will also receive this charge if you cancel an appointment less than 48 hours from the appointment. For more information regarding this please read my "new patient letter" or the consent form you signed.
